For current patient's of Dr. Krempowich ONLY

I now offer a "cancellation/quick call list" which allows me to contact people on a non-urgent basis for quick questions (less than 5 minutes) by phone when I have openings during the day. This is best used by patients who have a flexible schedule and whose issue is appropriate for a phone call.

It is available Monday to Friday, excluding holidays. No guarantees are made that you will be called, but I usually will be able to. If you do not receive a call by 5pm on the day you register, it is unlikely you will receive a call back that day. Please register again the next day that you are available.

If you need more than 5 minutes, please book an appointment instead using online booking. If I deem your issue too complex for the cancellation list or requires an in person exam, I will let you know to book an appropriate appointment instead. If your issue is urgent, contact the office directly.

If you would like to be placed on today's cancellation/quick call list, please fill out the form below, or email drkrempowich@gmail.com with the subject line "cancellation list" and a brief reason for the visit in the message (eg, prescription refill, result review, medication change check in etc), and the phone number you want me to call you at today. Please do not include sensitive medical information, as this is not a secure method of health information transmission. 

If I call and am unable to get ahold of you, this does NOT count as a no-show. However, if this occurs on 3 occasions of using the cancellation list service, I will not allow you to use the cancellation list service in the future. Please make sure you are by your phone, it is not on silent and you can pick it up when I call - it may be the only chance I have to call that day!
